Fludrocortisone in Septic Shock
The Surviving Sepsis 2021 Campaign makes a weak recommendation for the addition of steroids in patients with ongoing pressor requirements. They provide a recommendation for IV hydrocortisone 200 mg per day based on the ADRENAL, VANISH, and the APROCCHSS trials. The APROCCHSS trial showed a mortality benefit when hydrocortisone and fludrocortisone were added in septic shock versus placebo. The role of fludrocortisone was not addressed in the 2021 Surviving Sepsis Campaign; however, newer trials have hinted at potential benefit when added to hydrocortisone. The goal of this pharmacotherapy rounds is to assess the possible utility of fludrocortisone in septic shock.
